Appointments are required for all offender registrations.  Appointments may be made by either calling 815-235-8257; or by filling out the below form.  Upon receipt of the online form, someone with the Offender Registration Unit will contact you to schedule an appointment.  You must provide a working telephone number and a valid email address.  Please note that submissions of the online form are only monitored Monday - Thursday during normal business hours.  For additional information about the offender registration process, please visit
